Your First Visit

At your first appointment, Jericho Dental Centre requires some vital information to help us aid you in your dental treatment. Your medical and dental history will be reviewed during our examination. Dr. Shah will help to explain any areas of concern with x-rays and digital pictures of your teeth. She will then explain your treatment plan in detail, answering any questions you may have. Depending on your situation and dental needs, a second appointment may be necessary to finish your cleaning.

Appointments

To ensure our office is efficient, professional and able to meet all your needs, we require all appointments to be booked into the schedule, with the exception of emergencies.

We do require forty eight hours notice if you’re planning on rescheduling you’re appointment. There is a charge of $50.00 per hour booked for “short notice”, cancellations or missed appointments. When you fail to provide enough notice, another patient in need of dentistry and waiting for an appointment is prevented from receiving treatment. The forty eight hour notice allows other patients to rearrange their schedule in order to attend the appointment in your place. This policy allows us to provide the best possible treatment to all patients.
 
Dental Insurance

As a service to our patients with dental coverage, we do directly bill insurance companies for all basic and preventative treatment. However, payment of the patients’ portion is required on the date of service. Furthermore, should the insurance company not pay for all of the procedure; the patient is responsible for the balance owing. Also, if it’s your first visit to our office, on an emergency type basis, payment upfront will be due on the date of service. We can then supply you with a claim form for you to submit at your discretion to your insurance company.

Please note that your insurance is a contract between you and the company providing you your benefits. Please also understand that our office deals with many other patients’ insurance plans, each with different variations and conditions. Therefore, it is impossible for us to know everything about your insurance plan. It is the patients’ responsibility to understand the limitations and restrictions of their plan. Due to new privacy policies, many insurance companies won’t disclose information to us regarding patient’s coverage. This is why we require you to bring your insurance information with you upon the first appointment. If the proper insurance information is not supplied, both primary and secondary plan (if applicable), we will require full payment of the treatment performed on the date of service.

Fees and Payment

Our fees are based on the current fee guide, with the exception of major treatment; we do charge a set fee. We accept cash, MasterCard, Visa or Interac as forms of payment. For all major restorations, or treatment involving a lab fee, our office does require payment upfront on the date of service. We then submit the claim to your insurance company so that you can be reimbursed directly.
